Instead of evaluating function only inside a clinic, you'll work with patients in their own homes, where mobility limitations, fall risks, caregiver needs, equipment challenges, and everyday barriers become much easier to understand and address.

You'll have meaningful autonomy. Therapists organize their own patient appointments, manage their caseload, and make clinical decisions while coordinating with physicians, caregivers, hospital staff, and other members of the healthcare team.

The work is highly functional. You'll develop individualized exercise programs, evaluate durable medical equipment and adaptive equipment needs, help improve mobility and safety, and create treatment strategies based on what each patient needs to remain as independent as possible.

At the same time, you won't be practicing in isolation. You'll be part of a rehabilitation department that includes Physical Therapy, Occupational Therapy, and Speech-Language Pathology, giving you access to an interdisciplinary team while maintaining the independence that makes home-based therapy appealing.

What You'll Do

✔ Complete comprehensive physical therapy evaluations in patients' homes

✔ Develop individualized rehabilitation and home exercise programs

✔ Restore mobility, strength, balance, function, and independence

✔ Complete OASIS documentation for applicable therapy-only cases

✔ Assess durable medical equipment and adaptive equipment needs

✔ Coordinate care with physicians, caregivers, hospital staff, and other healthcare professionals

✔ Monitor clinical changes and communicate updates to the healthcare team

✔ Manage your own patient appointments and daily schedule

✔ Connect patients and families with appropriate community resources

✔ Provide supervision and mentorship to Physical Therapist Assistants when applicable

✔ Support safe transitions and optimal patient outcomes

Compensation & Benefits

Continuing education support includes an annual stipend and training opportunities incorporated into therapist schedules.

Minimum Qualifications

Required

✔ Graduate of an accredited Physical Therapy program

✔ Oregon Physical Therapist license in good standing

✔ Basic Life Support certification through the American Heart Association

✔ Valid driver's license

✔ Ability to work independently and travel throughout the local service area

Preferred

Doctoral-level physical therapy education

Experience with neurologic rehabilitation

Vestibular rehabilitation experience

Cardiac rehabilitation experience

Experience treating chronic pain
